The Tséhootsooí Medical Center Diabetes Program, alongside Nahata’Dziil Health Center, is committed to helping you gain the knowledge, skills, and confidence to thrive with diabetes.
We provide guidance and counseling to patients diagnosed with diabetes or at an increased risk of diabetes. We also work with your provider and the interdisciplinary care team to provide you with quality care. Our program is accredited by the Association of Diabetes Care & Education Specialists.
Yes, there is a full-time diabetes educator and part-time nursing assistant.
Always feel free to bring someone with you to your appointments. A family member or friend can help ask questions, remember the information your care team gives you, and provide support.

Your doctor or primary care provider can refer you. Ask them for a referral.
As a patients diagnosed with diabetes, you should be provided with information to improve your health. TMC offers access to a dietitian to help you manage food, a clinical pharmacist to help you manage your medications, and a diabetes educator to help you understand diabetes and manage your blood sugar.
Managed diabetes is when you have an A1c below 7% for most people and working with your provider to keep you healthy. Unmanaged diabetes is when your A1c is above 8% and you are not participating in your healthcare.
